Your health information is confidential, however de-identified case notes may be used for education, teaching, research or examination purposes. Cumulative results from case files may be used for research purposes, however, individual cases will not be used for case report publications without your prior specific written consent.
We may use third party services such to prescribe and dispense items such as supplements, herbal tonics, creams, flower essences or homeopathic remedies or order functional/pathology testing. Information such as name, address, email and/or phone number may be shared. Please notify us if you do not wish to have your information shared.
